Failure to Diagnose

Most mistakes in the emergency room result from failure to run the right tests. For example, in the case a heart attack, if someone complains of chest pains, the staff is supposed to order an EKG or a stress test. If that is done, the tests will point out the problem and it can often be fixed before permanent damage is done.

Sometimes, however, the right tests are not done. Because timely treatment is crucial to a good outcome for the patient in an emergency room, this lack of proper testing can have very serious consequences. A patient with chest pain may be discharged too early, when a heart attack could have been prevented.

Lost in the Shuffle
